Support Coordination WA is a fully independent company, offering leading NDIS registered support coordination and recovery coaching services across the metropolitan area of Perth and beyond.
Our dedicated team will support you to understand and implement your NDIS plan. We will inspire you to set goals that will help you reach your potential. Working together, we can help you to thrive, get the most from your plan and live your best life.
Support Coordination
This support will assist you to build the skills you need to understand, implement and use your plan. We will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Specialist Support Coordination
This is a higher level of support coordination. It is for people whose situations are more complex and who require specialist support. We will assist you to manage challenges in your support environment and ensure consistent delivery of service.
Psychosocial Recovery Coaching
Recovery coaches support participants to take more control of their lives and to better manage the complex challenges of day-to-day living.
Recovery coaches will work collaboratively with participants, their families, carers and other services to design, plan and implement a recovery plan, and assist with the coordination of NDIS and other supports. New Paragraph
